I understand that Dancebox Live will involve physical dance activity, and I confirm that I am fit and able to take part. I understand that participation is voluntary and that, as with all physical activity, there is a risk of injury.

I agree that I will take part responsibly, listen to instructions given by teachers and event staff, and stop participating immediately if I feel unwell, injured, dizzy, faint or unable to continue safely.

I understand that it is my responsibility, or the responsibility of my parent/guardian if I am under 18, to disclose any relevant medical conditions, injuries, allergies, medication or additional needs that may affect my participation.

I give permission for Dancebox staff, event staff or appointed first aiders to seek or provide appropriate medical assistance in the event of illness, accident or injury during the event.

I understand that Dancebox Studios & Theatre Works, its staff, teachers, guest faculty and event partners cannot be held responsible for injury, illness, loss or damage unless caused by proven negligence.

I confirm that I, or my parent/guardian if I am under 18, have read and understood the above and agree to take part in Dancebox Live under these conditions.
